System report name: ExamCard (or ExamCard1 to produce same information without the photograph)

General description: Card to go on candidate's table for an exam. Includes a photo so invigilators can ensure correct pupil is present.

Fields: Candidate number, full name, exam name, photo, UCI, date of birth, form and centre no. (Centre no. included for the candidates benefit as they often have to fill this in on their answer paper).

Normally launched from:Exams Officers Menu, reports tab, and complete the Examination Card area in the bottom right of the form. 

Other locations: To access this outside of the Exams Officers menu, set up the ExamCard form on a main menu.
