System report name: CheckSheetStyle7

General description: One of several very similar check sheets - this contains minimal information with space for the recipient to complete additional information which will then need to be added to the pupil record if appropriate.

Fields: One page per pupil, shows pupil information: full name, year, form, house, date of birth. Parent information name, address, home phone number, and email. Then spaces for remaining information to be completed.

Normally launched from: Pupil Finder, General

Other locations: Check sheet button on a pupil record (this defaults to CheckSheet, would have to be set up individually for your school if you wish to use a different Check Sheet style).
