General description: One of several very similar check sheets - a reduced checksheet (no pupil information) is produced for a divorced parent.

Fields: One page per pupil, shows pupil information: Chosen name, forename, middle names, surname, date of birth, ethnic group, form, year, house, previous school, student mobile, religion, passport number and expiry. Place of Birth, R.G. and C.P.F display the contents of the Pup Special 3, 1 and 2 fields in the Pupil - Miscellaneous tab. Siblings in the school. Parent information name, relationship and email for father and/or mother, check boxes for primary, report needed, fee payer, allow circulation, custody and admissions, address and all phone numbers with description and contact order, and details of employment.

Other locations: Check sheet button on a pupil record (this defaults to CheckSheet, would have to be set up individually for your school if you wish to use a different Check Sheet style).
