Fields: Front shows school logo and title, valid dates, pupil name, address, form, phone number and photo. Back has the following text: For your journey - take a seat and remain in it - fit the seat belt and keep it on - talk but do not shout - do not eat or drink - respect the comfort of all other travellers - do not distract the driver - take all you belongings with you - leave no litter. Report prints front and back side by side, four passes per sheet.
