General description: One of 5 very similar check sheets, this one includes medical conditions and allergies.

Fields: One page per pupil, shows pupil information: Chosen name, forename, middle names, surname, date of birth, ethnic group, gender, date of entry, form, year, house, previous school, student mobile, religion, boarding status, passport type. Medical conditions including medical alerts in red, allergies, medical practitioner and GP contact number. Parent information name, relationship and email for father and/or mother, check boxes for primary, report needed, fee payer, allow circulation, residence and admissions, address and all phone numbers with description and contact order. This one shows first names in title and Times New Roman font.

Normally launched from: Pupil Finder, medical lists or general.

Other locations: Pupil selections in Finder are very useful, so unlikely to be used elsewhere.
