This form is accessed from the Medical button on the main pupil form and gives access to key Medical information which you are likely to need if you are teaching a pupil or taking them away on a trip. (The button may be switched off in your school for data protection reasons.) If there is a serious medical condition this should be highlighted by a medical alert on the pupil record.
