This form should be made available in Lookup Tables or Lookup Forms. It is used to populate the list from which extra curricular activities are chosen to be added to the Extra Curr. tab of a pupil record.

The columns are as follows:

• Activity: the name of the activity
• Type: which type of activity is it? This is selected from the Extra Curric. Categories list, which is available from Lookup Tables.
• Active: should be ticked if the activity is currently available.
